In order to certify patients for medical cannabis in Connecticut, you’ll need to inform the state through the Department of Consumer Protection’s online form. Through that form you’ll assert that the patient has been diagnosed with condition eligible for palliative use of cannabis and that, based on a medically reasonable assessment of the patient’s medical history and medical condition, the potential benefits of palliative use of cannabis would likely outweigh the health risks.
Before certifying patients for marijuana, you will be required to register with the Connecticut Prescription Monitoring and Reporting System (“CPMRS”) so that you can have a complete picture of your patient’s controlled substance use, including prescriptions by other providers.
